I suspect it's photoshopped since the last finger is so huge compared to the others.

Its not photo shopped, People being born with one big finger rather than two of normal size is a lot more common than you think. My best friend has only 3 fingers on his left hand. Knew him for 2 years before I noticed.
Lol I did the same thing with a friend of mine, except it took me a year to notice he had only 3 fingers on one hand and was missing a knuckle from a couple others. I only noticed because he could draw the coolest ICP pictures ever and I was watching him draw one day and noticed he held the pencil kind of funny, because he didn't have the top knuckle on his pointer finger or his ring finger. But he did on his middle (didn't have a pinkie); it was kind of strange.
